Why is it hard to reach our goals despite being motivated when setting them?

For most people, failure is scary and there is no getting around it. The fact is that most people have been so conditioned into feeling like failing at something means that they are a failure as a human being. As a result, we simply do not try to meet our goals with sincerity. 

This fear undermines their self-worth, self-esteem and self-love. I blame school because it's 12 years of being in a situation where failure is seen as a bad thing, which means we learn to measure our worth based on how often we can avoid failing.
